How to Report Complaints and Adverse Events
To report a complaint, including any adverse events, on Olympus and OSTA products:
- To report a complaint, including an adverse event or troubleshooting request,
call TAC at (800) 848-9024, Option 1.
- To request service for a device that can be sent in for repair,
call Customer Solutions at (800) 848-9024, Option 3.
- To report a complaint by email, we prefer that you complete and submit a Customer Problem Report to firstname.lastname@example.org. However, we will also accept an e-mail without a Customer Problem Report if you state the facts and give us the customer information. Click here to access the Customer Problem Report form.
